Introduction
   

Tension Lipidosis:

This is a case of tension lipidosis. The small adhesion to the hepatic capsule had placed constant tension on the capsule, resulting in reduced blood flow to all the hepatocytes within the affected zone. Relative hypoxia of these hepatocytes in turn caused reduction in the rate of ATP dependent metabolism. As synthesis of proteins for export of lipid (as lipoproteins) out of hepatocytes is an energy-consuming process, triglyceride fats accumulated within the compromised cells .

The damage to the hepatocytes is potentially reversible if the tension on the capsule is relieved.

Fatty change is histologically distinctive and relatively easy to identify in H&E-stained tissue sections. However, an oil-red O stain or Sudan black stain could be used on frozen tissue sections to confirm the presence of lipid.
